Health Coach journeys: Your path to better health
You may be invited by the practice to take part in a Health Coach journey – a simple app-based programme to help you take practical steps towards better health.
What is a Health Coach journey?
They’re digital health plans that give you personalised tips in different areas of health, such as weight or blood pressure management.
They’re delivered through our trusted digital partner, Evergreen Life, to support you in improving key aspects of your health.
How to take part?
You can only access these journeys when invited by your clinician (either by email, SMS, or through the digital triage system). You’ll get clear instructions on how to get started and what to do at each stage.
- If invited, you’ll complete a short form to confirm eligibility.
- If eligible, you’ll be able to continue your journey in the Evergreen Life app.
- In the app, you’ll get helpful content and guidance over the next several months on improving aspects of your health.

- Managing BMI and body fat
- Blood pressure control
- Quit smoking support
- Lung health for those with asthma or COPD
- Blood sugar control and diabetes management
- IBS management
